See the readme. The app is fairly self explanatory, but there are some advanced features.

Basically, setup which civs you want to allow, how many players, whether to allow repeated civs, etc.

Hit the Go button outside the game or use the hotkey (CTRL+ALT+Space) in the game, then paste (CTRL+V) the generated list into the game's chat window.

Updated to 1.0.4

Changes: Installer, hotkey editor

That got me on an old idea of a random civ chooser ingame :D = game randomly chooses any of the available HC's at a start of a skirmish/lan game, so that one never know which of one's HC one will be playing.

Is that something you could code? :)

how it works? im so bad with mods so when i try to launch the game and play skirmish , i cant choose otto's a second time plz help

The generator knows nothing about ESO, individual players, or games. It works by social contract. The host generates the list and pastes it into the ESO hosted room's chat. All players then change to their assigned (and randomly selected) civs.

It's not a mod to the game, it's an app that runs outside of the game but makes creating random civ games on ESO pretty easy.

It isn't really what I expected from it.

Additional Comments:
I think it is better to have an ingame random civ generator, that you can use at skirmish or LAN.
