Most routers are configured through a web site located at the name of the local IP address they invent for themselves. This is usually 192.168.2.1 or 192.168.1.1. This would bring up a password page. After entering the password you would go through some menu of settings to open the above ports.
If your router is fancy and wants a "trigger port" for each open range of ports, use the following settings:
28800 (UDP) as a trigger port for 2300-2400, 47624 (TCP)
28800 (UDP) as a trigger port for 2300-2400, 47624 (UDP)
47624 (TCP) as a trigger port for 2300-2400 (TCP)
47624 (TCP) as a trigger port for 2300-2400 (UDP)
6667 (TCP) as a trigger port for 28800-29000 (TCP)
For some routers (mine, for example) this still will not allow you to host a game on the internet. But you should be able to join a game.
If you have a router the game's internal "What is my IP?" function does not work. It tells you the local IP your router assigns your computer instead of the WAN IP your router uses to communicate with the internet.
